Family Medical Clinic RN Case Manager

**SIGN ON BONUS: FT RN - $5000 EXPERIENCED/$3000 NEW GRAD**

Job Summary: The RN Case Manager is responsible for coordinating and managing patient care within the clinic setting to ensure patients receive appropriate, timely, and effective healthcare services. This position works closely with physicians, nursing staff, patients, families, and outside healthcare providers to coordinate care, monitor patient progress, facilitate referrals, and support continuity of care.

The RN Case Manager also assists with patient education, care planning, follow-up, and identifying barriers to care while promoting quality outcomes and patient satisfaction.

Qualifications and Job Requirements:Must hold a valid and active license in good standing as an RN in the state of Arkansas. Previous case management experience preferred. Must have BLS within 90 days.
