Responsibilities

• Develop and implement individualized treatment plans based on comprehensive assessments of patient needs and goals.
• Conduct thorough evaluations to determine patient mobility, strength, flexibility, and functional limitations.
• Deliver evidence-based physiotherapy interventions.
• Manage patient progress through regular re-assessments and modify treatment plans accordingly to optimize outcomes.
• Document all patient interactions accurately following clinical documentation standards and maintain detailed records in electronic health records (EHR) systems.
• Collaborate closely with physicians and other healthcare professionals to coordinate care plans and discharge strategies.
• Assist with discharge planning by providing education on home exercises, community resources, and ongoing care needs for optimal recovery post-treatment.
• Engage in continuous professional development to stay current with physiotherapy techniques related to orthopedics.

Qualifications

• Valid license or registration as a licensed Physical Therapist in the State of NY.
• New graduates are welcome!
• Proven experience practicing in outpatient rehabilitation centers or outpatient clinics with a focus on patient care.
• Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques relevant to outpatient care.
• Experience working with diverse populations including geriatric patients, pediatrics, neurological conditions, and orthopedic injuries.
• Familiarity with clinical documentation standards and proficient management of electronic health records (EHR) or electronic medical records (EMR) systems. We use WebPT.
• Demonstrated ability to develop treatment plans that incorporate discharge planning and community-based care strategies.
• Excellent client communication skills to educate patients effectively about their treatment process and home care routines.

Pay: $70,000. 00 - $105,000. 00 per year
