Physical Therapist

We are an established, Medicare-certified home health agency seeking a compassionate, motivated, and experienced Physical Therapist (PT) to join our growing team!

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled physical therapy services to homebound patients.
• Complete start-of-care assessments, reassessments, recertifications, and discharges as appropriate.
• Collaborate with nurses, physicians, and other healthcare professionals to coordinate patient care.
• Educate patients and caregivers on exercises, mobility, fall prevention, and home safety.
• Maintain accurate and timely clinical documentation in accordance with Medicare and agency requirements.
• Manage an assigned patient caseload while maintaining established productivity expectations.

Qualifications

• Current, unrestricted Florida Physical Therapist license.
• Previous home health experience preferred.
• Knowledge of Medicare home health guidelines and OASIS documentation preferred.
• Strong organizational, communication, and time-management skills.
• Ability to work independently while collaborating with a multidisciplinary team.
• Valid driver's license, reliable transportation, and ability to travel within the assigned service area.
