Join Our In-House Rehabilitation Team!

La Bella of Alton is seeking a compassionate, motivated, and skilled Per Diem Occupational Therapist (OT) to join our dedicated in-house rehabilitation team. This is an excellent opportunity to make a meaningful impact by helping residents regain independence and improve their quality of life in a collaborative subacute rehabilitation and skilled nursing environment.

As a member of our interdisciplinary team, you will evaluate, develop, and implement individualized treatment plans that promote functional independence, safety, and successful discharge outcomes.

Responsibilities

  • Evaluate residents' functional abilities and develop individualized occupational therapy treatment plans.
  • Provide skilled occupational therapy services to residents in a subacute rehabilitation and skilled nursing setting.
  • Establish functional goals and modify treatment plans based on resident progress.
  • Educate residents, families, and caregivers regarding treatment programs, adaptive equipment, and home safety recommendations.
  • Collaborate with physicians, nursing staff, physical therapists, speech-language pathologists, and other interdisciplinary team members to coordinate resident care.
  • Complete accurate, timely, and compliant documentation in accordance with facility policies and regulatory guidelines.
  • Participate in care conferences and discharge planning as appropriate.
  • Maintain productivity standards while delivering high-quality, resident-centered care.
  • Ensure compliance with all federal, state, and facility regulations.

Qualifications

  • Current Illinois Occupational Therapist license in good standing.
  • Graduate of an accredited Occupational Therapy program.
  • Current CPR certification (preferred).
  • Previous experience in skilled nursing, subacute rehabilitation, or long-term care is preferred, but new graduates are encouraged to apply.
  • Strong communication, documentation, and organizational skills.
  • Ability to work independently while collaborating effectively within an interdisciplinary team.
  • Passion for providing exceptional, resident-centered care.

Schedule

  • Per Diem (PRN)
  • Flexible scheduling based on facility needs
