RN-Case Manager-Hospice

Position: RN-Case Manager-Hospice Location: Sunnyvale CA 94085 Shift: Days: 8:30am-5pm (Schedule can be flexible based on patient's availability.)

Weekends: One weekend per month. Duration: 13 weeks License & Certification: RN CA License, BLS Pay Information: $2500-$2540/Week for 40 Hours

Job Summary:

The Hospice Nurse admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. They develop an individualised plan of care based on their assessment and document their observations and interventions related to the patient's environment.

This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.

Territory Details

Oakland, Alameda, San Leandro, Ashland, Castro Valley, San Lorenzo, Hayward, Union City, Fremont, Newark, Milpitas and Neighboring Cities as needed.

Required: 1 Year of Hospice Nursing Experience

Pay: From $2,550.00 per week
