Physical Therapist

Job Overview

As a Home Health Physical Therapist, you’ll deliver physiotherapy in patients’ homes and communities, helping people build mobility, manage daily activities, and progress through rehabilitation. You’ll assess each patient’s needs, develop individualized treatment plans, and coordinate care with patients, families, and healthcare professionals.

Responsibilities

• Evaluate patient mobility, strength, function, and home environment; apply anatomy, physiology, and medical terminology in clinical decisions.
• Develop and update care plans and treatment plans; provide physical therapy interventions, including trigger point therapy when appropriate.
• Deliver patient care and rehabilitation for varied needs, including orthopedics, neurology, pediatrics, and geriatrics.
• Communicate clearly with patients and caregivers, collaborate with healthcare professionals, and support discharge planning.
• Complete physical therapy documentation in electronic health records (EHR) or electronic medical record (EMR) systems, following clinical documentation standards.

Experience

• Experience practicing in a physical therapy role and providing patient care is essential.
• Home health or home and community care experience is valuable; experience with geriatric patients is especially relevant.
• Acute care, inpatient, hospital, outpatient, clinic, outpatient clinic, or outpatient rehabilitation center experience is helpful, but not required.
• Familiarity with patient rehabilitation, care plans, treatment plan development, and EHR/EMR management is preferred.

Pay: $125,000.00 per year
