Penndel Home Health & Nursing Services (PHHNS) is growing, and we’re looking for a Per Diem Physical Therapist to help patients improve mobility, regain independence, and remain safe in their homes.

Compensation & Service Area

• $65–$100 per visit, depending on visit type. Start of Care visits are compensated at a higher rate than routine visits.
• Per diem position with flexible scheduling.
• Patient visits throughout Bucks, Montgomery, and Philadelphia Counties.
• Office based in Langhorne, PA. Tell us your availability and preferred coverage area when applying.

Your Role

• Evaluate patients’ strength, balance, mobility, and functional abilities.
• Develop individualized treatment plans and provide skilled physical therapy.
• Help patients improve independence and safety with daily activities.
• Educate patients and caregivers on exercises, mobility, and safe movement.
• Communicate with the care team and complete timely clinical documentation.

Qualifications

• Active Pennsylvania Physical Therapist license.
• Strong clinical judgment, communication, and organizational skills.
• Ability to travel to patient homes within your assigned coverage area.
• A compassionate, dependable approach to patient care.
• Home health experience strongly preferred.
• WellSky/Kinnser experience is a plus.
