Skilled Nursing / Assisted Living Nurse Practitioner

Job Summary

JET Healthcare is looking for a compassionate and experienced Nurse Practitioner to join our growing team. We care for seniors where they live and aim to make every visit meaningful. Duties

• Complete thorough assessments and create individualized care plans
• Diagnose and treat acute and chronic conditions
• Manage and prescribe medications
• Monitor progress and adjust plans with the care team
• Educate patients and families in a kind, approachable way
• Provide preventive care and health screenings
• Maintain accurate documentation and follow regulatory standards
• Communicate closely with families, facility partners, and care teams
• Stay current with best practices in geriatric care

What We’re Looking For

• Master’s or Doctoral-prepared NP with an active Illinois license
• Illinois Controlled Substance and federal DEA registrations to prescribe controlled substances, or willingness to obtain both upon hire
• At least 1–2 years of experience in geriatric, adult, or palliative care
• Comfortable working in senior living settings: SNF - sub- and post- acute or ALF
• AGNP, FNP, or DNP or equivalent experience
• Excellent communication, empathy, and relationship-building skills
• Strong organizational habits and reliability
• Familiarity with ICD/CPT coding for billing purposes
• Commitment to delivering high-quality, patient-centered care
• Reliable and thorough progress notes documentation
