About this role
About us
iVitality MD is a boutique wellness and functional medicine clinic in Houston, founded alongside The Kidney Holistic Institute by Dr. Bismah Irfan, a double board-certified physician in internal medicine and nephrology and the author of Your Kidneys Can Heal.
We don't do "watch and wait." We look past the surface to find the root cause and treat the whole person — advanced testing, hormone optimization, IV and infusion therapies, peptides, and genuinely personalized care.
And we're growing right now. We've signed the lease on a clinic nearly double our current size, in our existing Houston location, and build-out is underway for an opening this winter. The new space is purpose-built for two therapies almost no one in Houston offers: therapeutic plasma exchange and colon hydrotherapy.
Those join what we already run every week — EBOO, IV ozone, HBOT, HOCATT, chelation, lymphatic drainage, frequency-specific microcurrent, PEMF and bioresonance.
Whoever takes this role walks into the new clinic as it opens and helps decide how it runs.
About you
You're a Nurse Practitioner or PA who lights up working directly with patients, and functional and integrative medicine is the work you actually want to be doing.
You're genuinely strong with IV and infusion therapy. That matters more here than in most clinics — EBOO and plasma exchange need reliable, large-bore access in two sites, often in patients whose veins aren't easy. If you can place a line under ultrasound guidance, say so clearly in your application.
Ideally you've managed hormones and peptides too. If you haven't done all of it yet but you're genuinely drawn to this medicine and want to learn it properly, tell us — we have the protocols and the physician time to train the right person.
A note on fit, worth reading
This role has two real sides. One is deep clinical work: hormones, peptides, IV and infusion medicine. The other is sitting with a patient and guiding them into the right level of care with us, which means being comfortable talking about programs and cost as part of a clinical conversation.
We never pressure anyone into anything. But if that side would feel like "sales" to you rather than advocacy, this isn't the right seat. If you love both, you'll do very well here.
What you'll do
- Establish and manage vascular access and run our infusion therapies — wellness drips, IV ozone, EBOO, chelation, and plasma exchange as we bring it online. You'll be the clinical owner of the infusion floor, working alongside our medical assistants.
- Interpret labs and manage hormone therapy end to end — estrogen, testosterone, progesterone, pellets, injections.
- Prescribe and manage peptide protocols alongside lifestyle medicine, nutrition and detoxification.
- Run new-patient consultations using our established framework. Dr. Irfan sits in with you at first; you run them independently as your track record builds.
- Own your patient relationships through follow-up. Our patients stay with us for years.
- Grow into leading group program sessions and taking a leadership role in the new clinic.
Requirements
- Active Texas NP or PA license, or license-eligible in Texas and able to complete endorsement before your start date
- Active DEA registration, or able to obtain one within 60 days — this role prescribes hormones and related therapies
- Strong, independent hands-on IV and infusion skill, including difficult access. You'll be placing lines, not only supervising them.
- A genuine interest in functional, integrative and root-cause medicine
- Comfortable with real autonomy and with patient-facing conversations about care plans
- No visa sponsorship at this time
A significant plus
- Ultrasound-guided vascular access
- Apheresis, plasma exchange, dialysis or infusion-center experience
- Hands-on hormone replacement therapy experience
- Peptide and GLP-1 prescribing experience
- Functional or integrative medicine training (IFM, A4M, ACAM or similar)
- Background in ER, critical care, infusion, med spa or family medicine
Compensation and benefits
- $141,000 – $156,000 per year, full-time W-2, with increases as you take on independent consults and group program leadership
- Health insurance stipend
- 401(k) with company match
- Retention bonus
- All federal holidays paid, plus 10 days PTO per year after a 90-day probation
- Functional medicine training support may be considered after 12 months with us
How to apply
Apply here with your resume and answer the questions in the application, including the short video.
The video is required. Two to three minutes, recorded on Loom (free at loom.com) or any shareable link. Introduce yourself and tell us why functional, root-cause medicine is the work you want to be doing.
We want to meet the human behind the credentials, and a resume can't show us that. Applications without a video link aren't reviewed.
No recruiters, staffing agencies or third-party vendors. We are hiring directly for this role and will not work with agencies. Please do not call the clinic — all applications and questions go through this posting.
