Job description

About this role

Integrated Care · integrated-care.org · Oahu, Hawaii

Integrated Care is hiring a Nurse Practitioner to lead a dedicated wound care service line. You will assess, diagnose, and manage complex wounds for patients referred from our own primary care providers and partner facilities, and you will see them where they are: in their homes, in care homes and respite centers, in clinic, and by telehealth.

Integrated Care is an Oahu-based practice delivering primary and urgent care, Psychiatric Mental Health Services, wound care and debridement, joint injections, Chronic Care Management, and Remote Patient Monitoring through mobile, in-home, telehealth, and in-clinic visits.

Our patients include residents of ARCH care homes and foster homes, members of Mental Health Kokua and the Institute for Human Services, and patients at Leahi Medical Respite. We accept all insurances except Kaiser, so visits carry no cost to the patient.

We are looking for a clinician who takes a proactive, evidence-based approach to healing, prevents complications in medically complex patients, and works well with a small, close-knit multidisciplinary team.

Responsibilities

  • Perform comprehensive wound assessments: etiology, size, depth, tissue type, exudate, periwound condition, and infection status, with photo documentation at each visit.
  • Build individualized treatment plans using advanced wound care techniques, including sharp debridement, dressing selection, offloading, compression, and skin substitutes where indicated.
  • Perform in-visit procedures such as debridement, suturing, wound closure, and other sterile procedures, using clinical supplies provided by Integrated Care.
  • Manage wounds complicated by diabetes, vascular disease, immobility, and behavioral health conditions; order and follow up on labs, imaging, and vascular studies as needed.
  • Coordinate care with our primary care and psychiatric providers, care-home and facility staff, case managers, and our physical therapy partners, and refer out for surgical or vascular evaluation when appropriate.
  • Document assessments, treatment plans, and progress notes in Tebra, and submit same-day orders (pharmacy, DME and supplies, labs, referrals) through our Provider Order Form so support staff can act on them.
  • Communicate with patients, families, caregivers, and facility staff through Spruce, and teach wound care protocols, infection prevention, nutrition, and lifestyle changes that support healing.
  • Share intake and progress notes with referring providers for continuity of care, and use accurate CPT and ICD-10 coding so visits bill cleanly.

Qualifications

Required

  • Active, unrestricted Hawaii APRN license with prescriptive authority (FNP or AGNP); current national certification
  • Active DEA registration
  • Proven experience in wound management, including sharp debridement
  • Current professional liability coverage, or eligibility under our group policy
  • Valid driver's license and reliable transportation for in-home and facility visits on Oahu
  • Ability to work under a collaborative practice agreement and in compliance with HIPAA

Skills

  • Skilled in physical examination, sterile technique, suturing, catheterization, and specimen collection and processing
  • Strong working knowledge of EMR/EHR systems; Tebra experience is a plus
  • Familiarity with CPT and ICD-10 coding for accurate, billable documentation
  • Clear, empathetic communication with patients, caregivers, and facility staff
  • Comfortable working independently in the field while staying closely connected to a remote support team

Preferred

  • Wound care certification (CWS, CWCN, WCC, or similar)
  • Critical care experience (ICU or Level I Trauma Center) for acute and complex cases
  • Experience with geriatric, behavioral health, hospice and palliative, or medically underserved populations
