Responsibilities

• Provide **Physical Therapy services to older adults** in home and community settings
• Evaluate patient mobility, strength, and functional limitations
• Develop and implement **individualized treatment plans**
• Deliver **evidence\-based therapeutic interventions** to improve movement, balance, and independence
• Maintain accurate **Medicare\-compliant documentation**
• Manage your own **schedule and caseload effectively**

Ideal Candidate

We’re looking for clinicians who are comfortable working **independently in a home health setting** and who are organized, motivated, and patient\-focused.

Preferred experience:

• **Medicare documentation and billing**
• Understanding of the **8\-minute rule and CPT coding**
• Experience managing scheduling and patient caseloads

Requirements

• **Active Physical Therapy license** in the state(s) of practice or eligibility to apply
• **Basic computer and documentation skills**
• **Current CPR certification**

Job Type

Full\-time \| Part\-time \| Per Diem opportunities available (W2 and 1099 available!)

Work Setting

• Patient homes within the community
• Community facilities such as Assisted Living Facilities and/or patient group homes
• Home health under Medicare Part B outpatient billing
