This is not a general DC posting.
Our immediate need is in Northwest Washington, DC.
We're looking for a PT who can accept patients in NW DC and help us meet the current demand for physical therapy services.
Tell us your availability and the areas within Northwest DC you're comfortable covering, and we'll work to match available patients with your schedule.
What You'll Do
- Complete physical therapy evaluations in patients' homes
- Develop individualized plans of care
- Provide therapeutic exercise, gait, balance, transfer, and mobility training
- Help patients improve strength, endurance, mobility, and independence
- Assess home safety and identify fall risks
- Develop and reinforce home exercise programs
- Educate patients, families, and caregivers
- Monitor patient progress and adjust treatment plans as needed
- Complete reassessments and discharge planning
- Collaborate with the interdisciplinary care team
- Complete accurate and timely documentation using WellSky
What We're Looking For
- Active Washington, DC Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Reliable transportation and ability to travel between patient homes
- Strong clinical and communication skills
- Excellent time\-management and organizational skills
- Ability to work independently in patients' homes
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
