Human Touch Home Health is looking for a Travel Physical Therapist (PT) for a 13\-week home health contract in Blacksburg, Virginia.

What You'll Do
**As a Travel Home Health PT, you will:**
- Complete physical therapy evaluations in patients' homes
- Develop individualized plans of care
- Provide therapeutic exercise, gait, balance, transfer, and mobility training
- Help patients improve strength, endurance, mobility, and independence
- Assess home safety and identify fall risks
- Develop and reinforce home exercise programs
- Educate patients, families, and caregivers
- Monitor patient progress and adjust treatment plans as needed
- Complete reassessments and discharge planning
- Collaborate with nurses, physicians, therapists, and other members of the interdisciplinary team
- Complete accurate and timely documentation using WellSky
What We're Looking For
- Active Virginia Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Valid Driver's License and reliable transportation
- Strong clinical and communication skills
- Ability to independently manage home health visits
- Strong time\-management and organizational skills
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
