Patient opportunities may be available throughout Washington, DC, including:

• Northwest DC
• Northeast DC
• Southeast DC
• Southwest DC

You do NOT have to cover the entire District.

The goal is to build a PRN caseload that works with your availability and preferred service area.

What You'll Do

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, mobility, and independence
• Assess home safety and identify fall risks
• Develop and reinforce home exercise programs
• Educate patients, families, and caregivers
• Monitor progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Collaborate with nurses, physicians, therapists, and other members of the interdisciplinary team
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Washington, DC Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Reliable transportation and ability to travel between patient homes
• Strong clinical and communication skills
• Excellent time\-management and organizational skills
• Ability to work independently in patients' homes
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
