Human Touch Home Health is looking for compassionate Physical Therapists (PTs) for PRN home health opportunities in Quakertown, Coopersburg, and nearby communities.

**Quakertown Area:** Quakertown, Richlandtown, Milford Square, Trumbauersville, Perkasie, Sellersville
**Coopersburg Area:** Coopersburg, Center Valley, Emmaus, Hellertown, and nearby communities
You do NOT have to cover the entire Upper Bucks or Lehigh Valley area.

PRN should actually feel flexible.
What You'll Do
- Complete physical therapy evaluations in patients' homes
- Develop individualized plans of care
- Provide therapeutic exercise, gait, balance, transfer, and mobility training
- Help patients improve strength, endurance, mobility, and independence
- Assess home safety and identify fall risks
- Develop and reinforce home exercise programs
- Educate patients, families, and caregivers
- Monitor progress and adjust treatment plans as needed
- Complete reassessments and discharge planning
- Collaborate with the interdisciplinary care team
- Complete accurate and timely documentation using WellSky
What We're Looking For
- Active Pennsylvania Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Valid Driver's License and reliable transportation
- Strong clinical and communication skills
- Ability to work independently in patients' homes
- Home Health Experience Preferred, but Not Required
- WellSky Experience is a Plus
