Patient opportunities may be available in Jim Thorpe, Lehighton, Palmerton, Weatherly, Nesquehoning, Summit Hill, Lansford, Albrightsville, and surrounding communities.

**And here's the important part:** You do NOT have to cover all of Carbon County.

What You'll Do

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, mobility, and independence
• Assess home safety and identify fall risks
• Develop and reinforce home exercise programs
• Educate patients, families, and caregivers
• Monitor patient progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Collaborate with nurses, physicians, therapists, and other members of the care team
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Pennsylvania Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to work independently in patients' homes
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
