Human Touch Home Health is looking for a compassionate Occupational Therapist (OT) for PRN home health opportunities in Quakertown, Coopersburg, and nearby communities.

We understand that PRN should mean flexibility. Whether you want to see patients around Quakertown or prefer to stay closer to Coopersburg, we'll work with your availability and preferred territory.

You do NOT have to cover the entire Upper Bucks or Lehigh Valley area.

What You'll Do

• Complete occupational therapy evaluations in patients' homes
• Develop individualized plans of care
• Help patients improve independence with activities of daily living
• Provide therapeutic interventions to improve strength, coordination, mobility, and functional ability
• Address dressing, bathing, grooming, toileting, meal preparation, and other everyday activities
• Assess home safety and recommend adaptive equipment or modifications
• Educate patients, families, and caregivers
• Monitor progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Collaborate with the interdisciplinary care team
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Pennsylvania Occupational Therapist License Required
• Graduate of an accredited Occupational Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical, communication, and organizational skills
• Ability to work independently in patients' homes
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
