About the Role

HouseCall MD is hiring a House Call Nurse Practitioner to serve patients in San Mateo, CA. You’ll bring primary care directly to patients in their homes, conducting assessments, making clinical decisions, documenting care, and coordinating with healthcare professionals to support safe, accessible treatment.

By identifying health needs and coordinating follow-up care, you’ll help patients and caregivers make informed decisions and maintain continuity of care.

What You’ll Do

• Complete patient intake, review medical records, assess vital signs, and perform physical exams and diagnostic evaluations
• Develop care plans, prescribe medications within applicable regulations and collaborative practice agreements, and provide health coaching
• Deliver home-based care, including medication administration, injections, specimen collection, and other procedures within your scope and training
• Coordinate referrals, case management, discharge planning, and follow-up with patients, caregivers, and healthcare professionals
• Maintain accurate documentation in the EMR/EHR, following HIPAA requirements

What You Need

• Active California Nurse Practitioner license and authorization to practice
• Prescribing authority that meets applicable DEA requirements
• Experience in primary care, home care, geriatrics, senior care, or a similar setting
• Strong patient assessment, triage, medication administration, communication, and documentation skills

Nice to Have

• Experience with children, assisted living, nursing homes, hospice, or post-acute care
• EMR/EHR experience (Epic, Athenahealth, Cerner, or eClinicalWorks)
• Knowledge of ICD and CPT coding
• Experience in acute care, emergency medicine, intensive care, dialysis, IV infusion, wound care, behavioral health, or clinical research
