Job Overview

We are seeking a Physical Therapist to provide individualized rehabilitation and support patients as they work toward improved mobility and function. The role includes assessing patient needs, developing treatment plans, and coordinating care across appropriate settings.

Responsibilities

• Evaluate patients and develop, implement, and update care plans based on clinical findings, goals, and progress.
• Deliver physical therapy and patient rehabilitation, including appropriate techniques such as trigger point therapy.
• Coordinate patient care with healthcare professionals and contribute to discharge planning, including transitions to home health or community care when appropriate.
• Maintain timely, accurate physical therapy documentation in accordance with clinical documentation standards and applicable electronic health record (EHR) or electronic medical record (EMR) systems.
• Communicate treatment recommendations and progress clearly with patients, families, and care teams.

Skills

• Knowledge of anatomy, physiology, medical terminology, and physiotherapy principles; experience practicing in a physical therapy role is relevant.
• Patient care experience, including work with geriatric patients; familiarity with pediatrics, neurology, or orthopedics is also valuable.
• Acute care, inpatient, hospital, outpatient, clinic, outpatient rehabilitation center, or home and community care experience is relevant.
• Ability to develop treatment plans, manage care plans, and support discharge planning across care settings.
• Strong client communication and documentation skills, including experience with EHR/EMR management.

Pay: $93,000.00 - $96,000.00 per year
