About the Role:

As a mobile provider, you’ll deliver comprehensive primary care to seniors in their private homes, assisted living facilities, and senior communities.

Responsibilities include:

• Conducting in-home assessments, diagnoses, and treatments
• Using Athenahealth EMR for accurate and efficient documentation
• Collaborating with a supportive clinical team
• Educating patients and families on wellness and chronic care
• Following safety and care standards across diverse settings

We’re Looking For:

• Licensed APRN or PA (Active and in good standing)
• Prior experience in geriatric or mobile care preferred
• Familiarity with assisted living, home health, or special needs populations a plus
• Proficient in EHR/EMR systems—Athenahealth experience preferred
• Compassionate, self-motivated, and reliable

Schedule:

• Monday to Friday (Weekends optional)
• Travel-based: This is an on-the-road position
