Integrum Medical is a growing Phoenix primary care and behavioral health practice serving adults and seniors through three settings: our clinic, on-site visits at assisted-living facilities and group homes, and telehealth. We’re looking for primary care Nurse Practitioners who want autonomy, variety, and flexible schedules.

What You’ll Do

• See patients at our Phoenix clinic, at partner assisted-living facilities and group homes, and through telehealth
• Manage chronic conditions such as diabetes, hypertension, COPD, CHF, and dementia
• Handle new patient evaluations, follow-ups, and acute visits
• Manage medications and coordinate care with facility staff, families, specialists, and our behavioral health team
• Complete accurate, timely documentation in our EHR

Requirements

• Active Arizona NP license (FNP or AGNP certification)
• Active DEA registration, or willingness to obtain one
• 1+ year of NP experience; geriatric, long-term care, or home-based care experience strongly preferred
• Reliable transportation for facility visits across the Phoenix area
• Strong, timely charting habits
• Self-directed, organized, and comfortable working independently
