**Job Summary:**

Homeside Rehab is seeking a compassionate and skilled Physical Therapist or Occupational Therapist to join our home health team. In this role, you will deliver individualized, evidence\-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

**Job Responsibilities:**

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence\-based PT/OT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

**Job Qualifications:**

• Active **RI** Physical Therapy License OR Occupational Therapist License — required
• Basic computer proficiency for electronic documentation
• Strong clinical assessment and interpersonal communication skills
• Ability to work independently in a home health environment
• Valid **RI** driver's license and reliable transportation for home visits
• Pediatric experience is preferred
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings

**Job Details:**

• Job Type: Full\-time, Part\-time, Per Diem
• Location: Providence, RI 02909
• Salary: $65 \- $80 per hour
• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
