About the Role:

HOMECARE RESCUE LLC is looking for a dedicated and compassionate Physical Therapist to join our team on a PRN basis in Wesley Chapel, FL. Responsibilities:

• Conduct comprehensive physical therapy evaluations and assessments in patients' home environments
• Develop and implement individualized treatment plans to restore function, mobility, and independence
• Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education
• Monitor and document patient progress, adjusting care plans as clinically appropriate
• Educate patients and caregivers on home exercise programs, safety, and fall prevention strategies
• Collaborate with interdisciplinary home health team members to ensure coordinated, holistic patient care
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards

Requirements:

• Active Florida Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or MPT)
• Home health experience preferred but not required
• CPR/BLS certification required
• Valid driver's license, reliable transportation, and willingness to travel to patient homes
• Strong clinical assessment, critical thinking, and problem-solving skills
• Excellent communication skills and a patient-first, compassionate approach to care
• Knowledge of Kinnser/Wellsky preferred, but willing to provide training.
