Job description

About this role

Are you an Occupational Therapist looking for PRN work in a facility where your clinical expertise genuinely changes lives? Home of the Innocents is seeking a PRN Occupational Therapist to join our therapy team at the Kosair for Kids Complex Care Center, Kentucky’s specialized pediatric skilled nursing facility for medically complex children and young adults.

A Day in the Life

Every day at Kosair for Kids Complex Care Center is different, and every resident brings unique needs. You'll conduct evaluations, implement evidence-based treatment plans, document progress, and collaborate with nursing, physical therapy, speech therapy, and the broader care team.

You might be working on fine motor skills with one resident, aquatic therapy goals with another, and conducting a transportation assessment for a third. You'll supervise COTAs, contribute to care plans, and build real relationships with the children and families you serve.

Schedule

PRN (as needed) — flexible scheduling. Days, evenings, nights, weekends, and holidays based on resident and staffing needs. On-site at Kosair for Kids Complex Care Center.

How This Position Contributes to Our Mission

  • Deliver occupational therapy services using evidence-based interventions: fine motor skill development, sensory processing, upper extremity range of motion, ADL training/retraining, aquatic therapy, assistive technology, and orthotics/splinting
  • Conduct comprehensive OT evaluations at admission, following significant changes in condition, upon return from hospitalization, and annually
  • Document OT services in the EMR including progress notes, certifications, recertifications, MDS assessments, Care Plan contributions, and discharge summaries
  • Conduct Resident Transportation Assessments and establish resident-specific transportation plans; train caregivers and guardians on transportation safety
  • Clinically supervise COTAs in accordance with licensure requirements, supporting them to achieve resident-centered goals
  • Collaborate with the interdisciplinary team — including Physical Therapy, Speech Therapy, and Nursing — to optimize functional outcomes and communicate effectively with legal guardians regarding progress and treatment plans

Skills and Qualifications

Required:

  • Master's or Doctoral degree in Occupational Therapy, OR 10 years of experience in the field of Occupational Therapy
  • Board-certified, unencumbered Occupational Therapist (OT) license in Kentucky
  • National Child Passenger Safety Certification, or ability to obtain within first year of employment
  • Current CPR/Basic Life Support (BLS) — provided by employer if not current
  • Strong interpersonal and motivational skills to build trauma-informed relationships
  • Proficiency with EHR documentation and clinical communication workflows
  • Strong analytical, clinical assessment, and critical thinking skills
  • Effective time management, prioritization, and teamwork in a fast-paced, 24/7 care environment

Preferred:

  • 6+ months working with pediatric or young adults with developmental and intellectual disabilities and/or complex medical needs
  • Experience in long-term care or complex care settings
  • Experience with aquatic therapy, upper extremity orthotics, assistive technology, and sensory processing treatment modalities

What You'll Receive as a Valued Member of Home of the Innocents

  • Competitive PRN hourly rate
  • Flexible scheduling — PRN allows you to work around your existing commitments
  • 401(k) available for eligible team members
  • Training and certification opportunities
  • Mission-driven culture — trauma-informed environment where every OT session makes a difference
