**Overview**
We are seeking a motivated and energetic Physical Therapist to join our team on a PRN basis serving the North/West Oakland County area. Patients may be seen in Novi, Commerce Twp, White Lake, W. Bloomfield, and surrounding areas.
This role offers an exciting opportunity to make a meaningful difference in patients’ lives by providing expert rehabilitative care across diverse settings, including outpatient clinics, hospitals, and home health environments. The ideal candidate will be passionate about patient care, possess strong clinical skills, and thrive in a dynamic healthcare setting. This position will grow with time as referrals increase.
**Responsibilities**
- Develop and implement personalized treatment plans for patients across various age groups, including geriatrics, pediatrics, neurology, orthopedics, and outpatient rehabilitation.
- Conduct comprehensive assessments to evaluate patient needs and establish appropriate therapy goals.
- Deliver evidence\-based physiotherapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and mobility training.
- Collaborate effectively with healthcare professionals—including physicians, nurses, and other therapists—to coordinate patient care and discharge planning.
- Maintain accurate and detailed clinical documentation adhering to healthcare standards and electronic health records (EHR) management protocols.
- Educate patients and their families on treatment plans, home exercises, safety measures, and community resources to support ongoing recovery.
- Participate in ongoing professional development to stay current with physiotherapy practices, medical terminology updates, and clinical documentation standards.
**Requirements**
- Valid state licensure as a Physical Therapist with practicing in a physical therapy role.
- Proven experience working with geriatric patients in outpatient clinics or home health settings.
- Strong knowledge of anatomy, physiology, orthopedics, neurology, and medical terminology relevant to physical therapy.
- Demonstrated ability to develop treatment plans tailored to individual patient needs while adhering to clinical documentation standards.
- Experience with inpatient hospital care or outpatient rehabilitation centers is preferred.
- Familiarity with EMR systems or electronic health records (EHR) management for accurate documentation of patient care activities.
- Excellent client communication skills with the ability to motivate patients through their rehabilitation journey.
- Ability to work independently in community\-based settings while collaborating effectively with healthcare teams.
