Home Health Registered Nurse

Job Overview

As a Home Health Registered Nurse, you’ll bring skilled, compassionate care directly to patients in their homes and communities. You’ll assess health needs, deliver and coordinate care, and help patients and caregivers understand each step of the care plan. Your work supports safer transitions, greater confidence, and continuity of care.

Duties

• Assess patients, monitor vital signs, review symptoms, and document findings in electronic health records (EHRs) while protecting privacy under the Health Insurance Portability and Accountability Act (HIPAA).
• Develop and update care plans; administer medications.
• Coordinate referrals, case management, intake, discharge planning, durable medical equipment (DME), and communication with clinicians, patients, and caregivers.
• Provide health coaching and education on nutrition, treatment plans, and symptom management; respond to changes in condition and follow emergency procedures.

Experience

• Required: Current registered nurse licensure for the applicable jurisdiction and the ability to provide patient-centered care in home and community settings.
• Relevant skills include patient assessment, nursing, medication administration, medical documentation, medical terminology, physiology and anatomy knowledge, and basic life support.
• Home care, home and community care, pediatric care, hospice and palliative medicine, long-term care, or post-acute care experience is valuable.
• Familiarity with electronic medical record (EMR) and EHR systems.

Experience with Cerebral Palsy DX, Pediatrics and Home health care a plus.

Immediate hire. 8 hours a week. Scheduled day is flexible.

1099 position. Weekly pay.

Pay: $55.00 per hour

Expected hours: 8.0 per week
