Compensation:

• $60 – $65 per hour

Position Summary & Responsibilities (SNF)

• Evaluate and treat SNF residents
• Document care; ensure Medicare/PDPM compliance
• Collaborate with interdisciplinary care team
• Support smooth facility startup and operations

Requirements

• Active Physical Therapist (PT) license
• SNF or post-acute rehab experience required
• Knowledge of Medicare/PDPM and documentation standards
• Ability to work in newly transitioned facility environment
