Physical Therapist – SNF Facility (Sacramento, CA 95825) (Contract)

Schedule:

• 13-week contract (ASAP start)
• 36 hours/week
• Travel or Permanent option

Pay:

• $90/hour

Position Summary & Responsibilities:

• Evaluate and treat SNF residents
• Develop and implement individualized PT treatment plans
• Provide gait training, therapeutic exercise, and mobility support
• Document patient status, progress, and outcomes
• Collaborate with interdisciplinary rehab team
• Support functional improvement and safe discharge planning
