Position Summary & Responsibilities:

• Evaluate and treat SNF residents requiring physical therapy services
• Develop and implement individualized treatment plans to improve mobility and function
• Perform ongoing reassessments and adjust care plans as needed
• Document evaluations, treatments, and progress in compliance with facility standards
• Collaborate with interdisciplinary team to support optimal patient outcomes
• Educate patients and caregivers on exercises, safety, and rehabilitation goals
