Schedule & Assignment Details:
- Location: SNF facility, CA 90804
- Start Date: ASAP
- Duration: 13-week contract
- Hours: 36 hours/week
- Pay: $85–$87/hour
- Setting: Skilled Nursing Facility (SNF)

Position Summary & Responsibilities:
- Evaluate and treat SNF residents requiring physical therapy services
- Develop and implement individualized treatment plans
- Improve patient mobility, strength, balance, and functional independence
- Document evaluations, progress notes, and discharge planning accurately
- Collaborate with interdisciplinary healthcare team to support patient care goals
- Educate patients and caregivers on therapy plans and safety strategies
