Physical Therapist – SNF Facility (CA 95129) (Contract)

Schedule:

• 13-week contract (ASAP start)
• 36 hours/week
• Travel or Permanent option available

Pay:

• $95/hour

Position Summary & Responsibilities:

• Evaluate, treat, and manage SNF patients
• Develop and implement individualized PT treatment plans
• Provide therapeutic exercise, mobility, and gait training
• Document evaluations, progress, and outcomes
• Collaborate with interdisciplinary care team
• Support functional recovery and discharge planning
