Position Summary & Responsibilities
- Evaluate patients; develop & implement individualized PT plans
- Provide therapy to restore mobility, strength, and function (SNF/Acute)
- Track progress; modify treatments as needed
- Maintain accurate, compliant documentation
- Collaborate with interdisciplinary healthcare team
