Position Summary & Responsibilities
- Provide PT evaluations and individualized treatment plans for SNF patients
- Deliver therapeutic interventions to improve mobility, strength, and function
- Monitor progress and adjust care plans as needed
- Document treatments and patient outcomes accurately
- Collaborate with interdisciplinary care team
