Responsibilities

• Conduct initial evaluations and ongoing reassessments to inform individualized treatment plans.
• Deliver physical therapy interventions using evidence\-based techniques and modalities.
• Collaborate with an interdisciplinary team to coordinate care and set patient\-centered goals.
• Maintain accurate, timely clinical documentation including evaluations, progress notes, treatment plans, and discharge summaries.
• Supervise assigned assistants and delegate clinical tasks as appropriate.
• Provide patient and family education on treatment plans, safety, and discharge planning.

Requirements

• Doctor of Physical Therapy (DPT) degree required; licensure in the state of practice required; license eligibility rules apply if obtained after January 2016 or with foreign equivalent.
• Physical Therapy licensure in the state of practice (Oregon) required.
• CPR or Basic Life Support (BLS) certification from an approved vendor required.
• Two years of Physical Therapy experience preferred.
• Ability to work in a rural care setting and meet the 4x10 schedule requirements.
