We are seeking a per diem Physical Therapist with good communication and organizational skills.

Hattie Ide Chaffee Home is a 69 bed non-profit nursing community, with our own in-house therapy team and newly opened 27 bed rehabilitation wing. We are top-rated, offer a warm, intimate environment, and pride ourselves on the care we provide to our residents and their families.

Must have an active RI License. Prior Nursing Home experience is required.

Overall Responsibilities

Responsibilities include, but not limited to, completing all documentation and billing requirements, perform resident assessments and evaluations to determine functional level and establish a therapeutic program. Develop a therapy plan of care to include goals and discharge planning. All aspects of the role must be in accordance with current Federal, State and Local Regulations.
