• Eastside Tucson
• Northwest Tucson

What You'll Do

• Manage a caseload of hospice patients and coordinate individualized care plans.
• Provide skilled nursing assessments, symptom management, and compassionate support.
• Educate and guide patients and families throughout their hospice journey.
• Collaborate with our interdisciplinary team to deliver exceptional care.
• Maintain timely, accurate documentation.
• Participate in the on-call rotation as needed.

What We're Looking For

• Active Arizona RN license.
• Minimum 1 year of nursing experience.
• Hospice, palliative, or home health experience preferred.
• Strong communication, organizational, and clinical skills.
• A compassionate, patient-centered approach to nursing.
