About Happy at Home Consulting

Happy at Home Consulting is a locally owned practice helping older adults and individuals with changing mobility needs continue living in their homes with greater ease and independence.

We provide outpatient physical, occupational, and speech therapy directly in clients’ homes, along with home accessibility consultations, caregiver education, and fall prevention services.

Our approach is practical and individualized. We focus on helping clients move more confidently, participate in meaningful activities, and make the most of their home environments.

About the Position

We are seeking a part-time Physical Therapist to join our growing team in the Des Moines metro area.

This position is ideal for a PT who enjoys working with older adults, values clinical independence, and wants the flexibility to provide individualized care without the pace of a traditional outpatient clinic.

Our services are provided under the outpatient therapy model, including Medicare Part B, rather than traditional home health. This allows us to focus on functional outcomes, mobility, fall prevention, and helping clients maintain independence at home.

We are also expanding our emphasis on evidence-based fall prevention programming and private-pay wellness services.

Responsibilities

• Provide individualized physical therapy evaluations and treatment in clients’ homes.
• Develop treatment plans focused on mobility, strength, balance, and functional independence.
• Identify fall risks and implement evidence-based interventions.
• Educate clients and caregivers on exercise programs, mobility strategies, and fall prevention.
• Recommend appropriate assistive devices and mobility equipment.
• Collaborate with occupational therapists and other members of the interdisciplinary team.
• Complete documentation in accordance with Medicare and practice requirements.
• Communicate with referral sources and other healthcare professionals as appropriate.

Qualifications

• Current Iowa Physical Therapist license or eligibility for licensure.
• Graduate of an accredited physical therapy program.
• Experience working with older adults preferred.
• Interest in fall prevention, functional mobility, and aging in place.
• Strong communication and independent clinical decision-making skills.
• Reliable transportation and willingness to travel to clients’ homes within the service area.
• Ability to manage documentation and scheduling responsibilities.

Experience in home health, outpatient therapy, geriatrics, or community-based rehabilitation is helpful but not required.

Schedule and Work Environment

• Part-time position with flexible daytime scheduling.
• No routine evenings or weekends.
• Services provided in clients’ homes throughout the Des Moines metro area.
