Primary Care Physician/ Nurse Practitioner/Functional Medicine

Job Summary

Provide thoughtful, evidence-informed primary care through a functional medicine approach. You’ll assess patients, develop individualized care plans, and coordinate with healthcare professionals to support prevention, diagnosis, and ongoing health needs.

Duties

• Conduct patient assessments, physical examinations, and diagnostic evaluations; review medical records, vital signs, and available medical imaging.
• Diagnose and manage common health concerns, including chronic conditions, and provide primary care, family medicine, and preventive services.
• Create care plans that may address nutrition, health coaching, diabetes care, immunizations, medication administration, and referrals to specialists.
• Document care accurately in electronic medical record (EMR) and electronic health record (EHR) systems, following Health Insurance Portability and Accountability Act (HIPAA) requirements.
• Collaborate with healthcare professionals across specialties, including cardiology, dermatology, gastroenterology, endocrinology, and behavioral health; support patient service and follow-up.

Requirements

• Physician, nurse practitioner, or other applicable advanced practice credentials and licensure required for the role and jurisdiction.
• Experience in primary care, family medicine, internal medicine, or an outpatient clinic; post-residency experience is relevant for physician applicants.
• Working knowledge of healthcare diagnosis, patient assessment, physiology, medical terminology, and care planning.
• Experience with EMR/EHR systems; Epic experience is a plus. Familiarity with medical office workflows, medical records, and coding is helpful.
• Preferred: Basic life support certification, telehealth experience, clinical research, or exposure to urgent care, geriatrics, or specialty care.

Pay: $81,100.00 - $97,800.00 per year
