Full-Time Home Health Physical Therapist
to join a growing and collaborative team serving patients throughout Bucks County/NE Philadelphia.

What You'll Do
As a Home Health Physical Therapist, You Will
- Evaluate patients in their homes and assess physical abilities and functional limitations.
- Develop individualized treatment plans based on each patient's goals and needs.
- Provide skilled physical therapy to restore, maintain, or improve physical function.
- Help patients manage pain and prevent further disability or functional decline.
- Educate patients and families on exercises, mobility, safety, and strategies for continued progress.
- Collaborate with interdisciplinary care teams to provide a coordinated patient experience.
- Monitor patient progress and adjust treatment plans as appropriate.
- Help patients achieve greater independence and quality of life.
Ideal Background
We are especially interested in Physical Therapists with experience in:
- Home health
- Geriatrics
- Long-term care / skilled nursing
- Other settings involving medically complex or older adult populations
Candidates coming exclusively from a traditional outpatient background may not be the best fit for this particular opportunity.
