Get Well Home Health, Inc. is a growing home health agency based in the Santa Clarita Valley, providing

compassionate, quality care to patients in their homes.

Job Description

• Perform initial and ongoing patient assessments (including OASIS) and develop individualized care plans
• Administer medications and treatments per physician orders
• Perform wound care, including cleaning and re-dressing as needed
• Measure and monitor vital signs, and identify/report changes in patient condition
• Coordinate with physicians, LVNs, HHAs, and other care team members on treatment plans
• Supervise and delegate tasks to LVNs and HHAs within their scope of practice
• Maintain accurate, compliant clinical documentation and progress reports

Qualifications

• Strong initiative to keep the agency in compliance
• Well organized, self-motivated, and able to work independently under the supervision of the Director of

Nursing

• Excellent organization, communication, and follow-up skills
• Excellent computer skills (EMR documentation experience preferred)

Requirements

• Valid CA RN license
• Valid CA drivers license

RNs at Get Well Home Health have a clear path to grow into leadership. Field RNs can move into a part-time QA

RN role, and from there into a full-time QA RN position overseeing chart review and compliance.

Pay: $35,000.00 - $100,000.00 per year
