Advanced Practice Practitioner (APP) – Chronic Kidney Disease Clinic Job Description – Georgia Nephrology

Location: Conyers, GA Schedule: Full-Time, Monday–Friday | Outpatient

Georgia Nephrology, the state’s largest private Nephrology practice, seeks a passionate and experienced Advanced Practice Practitioner (NP or PA) to join our nephrology practice and provide comprehensive care to both ESRD and CKD patients.

Responsibilities

• Evaluate, diagnose, and manage patients with ESRD and chronic kidney disease and related conditions.
• Develop and implement individualized treatment plans in collaboration with nephrologists.
• Monitor kidney function, medications, labs, blood pressure, and disease progression.
• Provide patient education focused on CKD/ESRD management, lifestyle modification, and preparation for renal replacement therapy when appropriate.
• Order and interpret appropriate laboratory and diagnostic testing.
• Document patient care accurately in the electronic medical record.
• Participate in quality improvement initiatives and support a patient-centered approach to CKD/ESRD care.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current Georgia NP or PA license and appropriate national certification.
• Nephrology, internal medicine, or chronic disease management experience preferred.
• Strong clinical, communication, and organizational skills.
