Form Psychiatry is a clinician-led telepsychiatry group on a mission to expand access to high-quality mental health care. We’re actively hiring NY‑licensed PMHNPs now. We partner with PMHNPs who seek flexible scheduling, fair and predictable pay, and full administrative support in an outpatient telehealth setting.

What You’ll Do:

• Provide outpatient psychiatric evaluations and medication management via secure video (telepsychiatry).
• Treat a steady stream of adult patients.
• Focus on care, not admin – our team handles scheduling, billing, and insurance credentialing for you.
• Maintain thorough documentation in our user-friendly EHR.

What You’ll Need:

• Active New York RN license and NYSED Nurse Practitioner certification in Psychiatry (PMHNP-BC), plus an active DEA registration. Multi-state licensure a plus.
• 3,600+ hours of qualifying NP practice (eligible to practice without a written collaborative agreement under New York’s NP Modernization Act).
• 3+ years of post-training psychiatric prescribing experience (preferred).
• A private, HIPAA-compliant home office (reliable internet, webcam) for telehealth.
• Professional liability malpractice insurance (or willingness to obtain coverage).
• Compassion, integrity, and a commitment to high-quality documentation and patient care.

Compensation & Schedule

This is a part-time, 1099 independent-contractor position with competitive, transparent compensation: a flat fee per completed visit, with no guaranteed minimum. Providers are paid semi-monthly (15th and last day of the month) via direct deposit. We require at least 20 patient-facing hours per week to ensure continuity of care.

No on-call duties. You have full control over your schedule – you set your available days/times in our scheduling system.
