JOB DESCRIPTION Occupational Therapist (OT) - Home Health
Qualifications and Requirements:
- Valid FL Occupational Therapist license
- Must have a valid FL Driver's License
- Must have current CPR Card
- Must have current AHCA Background Screening Report
- Must have either Chest XR report, 2 Step TST report or TB Lab Test Report (QuantiFERON Gold)
- Must have a Doctor's Certificate of Good Health
- Strong knowledge of home health regulations and best practices
- Excellent clinical assessment and critical thinking skills
- Ability to work independently and manage time effectively
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
- Proficient in using electronic medical record (EMR) systems
RESPONSIBILITIES:
-* This position will cover Citrus, Crystal River, Floral City* and surrounding areas
1. Evaluates patient’s functional status (muscle function, endurance, visual coordination,written and verbal communication skills, self care ability, work capacity, etc.) Evaluateshome environment for hazards or barriers to more independent living.
Identifies equipmentneeds. Participates in the development of the total plan of care.
2. For patients who plan to return to work, the occupational therapist may perform workcapacity evaluation and may refer to specialized vocational training in accordance withOrganization policy.
3. Develops treatment program and establishes goals for improved function. Communicatesplan of care to referring physician and other Organization professionals.
4. May teach new skills or retrain patient in once familiar daily activities that have been lostdue to illness or injury, in accordance with Organization policy.
5. May fabricate splints and instruct patient in the use of various types of adaptive equipmentto improve function.
6. May train patient in the use of prosthetic and/or orthotic devices.
7. Maintains appropriate clinical records, clinical notes, and reports to the physician anychanges in the patient’s condition. Submits these records in accordance with Organizationpolicy.
8. Maintains contact/communication with other personnel involved in the patient’s care topromote coordinated, efficient care.
9. Attends and contributes to inservices, case conferences, and other meetings as required byOrganization policy to ensure coordinated and comprehensive plans of care for the patientsof the Organization.
10. I dentifies patient and family/caregiver needs for other home health services and refersas appropriate.
11. Instructs patient’s family/caregiver and other Organization health care personnel in patient’streatment regime when indicated and appropriate.
12. Supervises certified occupational therapy assistants according to organization policy andstate regulations.
13. When therapy is the only skilled service, instructs, supervises and evaluates home healthaide care every two (2) weeks.
14. Participates in discharge planning for patient.
