Registered Nurse Case Manager

Job Summary

As a Registered Nurse Case Manager, you’ll coordinate patient care across hospital, outpatient, and post-acute settings. You’ll assess needs, guide discharge planning, and work with patients, families, and healthcare professionals to support safe, appropriate transitions.

Responsibilities

• Complete patient assessments, intake, and diagnostic evaluations; review vital signs, medical records, and electronic health records (EHR).
• Develop and coordinate clinical case management and discharge plans with patients and healthcare professionals.
• Apply utilization management practices, managed care guidelines, health regulation policies, and Health Insurance Portability and Accountability Act (HIPAA) requirements.
• Communicate clearly with clients and care teams, provide responsive patient service, and document care accurately.
• Support patients across acute, inpatient, outpatient, clinic, home health, and hospice settings, as appropriate to the role.

Experience

• Registered nursing experience and knowledge of patient care, physiology, nursing practice, and medical terminology.
• Hospital experience; critical care, intensive care unit (ICU), emergency room, medical-surgical, or acute care experience is valuable.
• Experience in hospital medicine, emergency medicine, primary care, pediatrics, geriatrics, behavioral health, infectious disease care, or post-acute care is helpful.
• Additional relevant experience may include Level I or Level II trauma centers, pediatric intensive care unit (PICU), operating room (OR), and inpatient or outpatient case management.
• Familiarity with EHR management, clinical case management, discharge planning, utilization management, and managing patients as a nurse case manager is preferred.

Pay: $45.00 - $60.00 per hour

Expected hours: 10.0 – 15.0 per week
