Home Health Physical Therapist

The Physical Therapist evaluates and treats patients to improve mobility, strength, balance, endurance, and functional independence while reducing pain and preventing further injury.

Responsibilities:

• Complete physical therapy evaluations and reassessments.
• Develop individualized treatment plans.
• Provide therapeutic exercises, gait training, balance training, and transfer training.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Monitor patient progress and modify treatment plans as needed.
• Maintain timely and accurate documentation.
• Comply with all agency policies and regulatory requirements.

Qualifications:

• Current Florida Physical Therapist license.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Home health experience preferred.
• Excellent clinical and communication skills.

Pay: $30.00 - $40.00 per hour
