Job Summary

F & B Care Therapy Solutions LLC is seeking a compassionate, dependable, and bilingual Registered Nurse (RN) to provide skilled nursing services to patients in their homes.

This is a flexible per diem opportunity for an RN interested in home health care. Patient assignments will be based on the nurse's location, availability, qualifications, and agency needs. Nurses will not be required to cover all counties; visits will be assigned within an appropriate service area whenever possible.

Responsibilities

• Perform skilled nursing visits and patient assessments according to physician orders and individualized plans of care.
• Complete Start of Care (SOC), Resumption of Care (ROC), recertification, reassessment, and discharge visits as assigned and qualified.
• Perform wound care, medication education, disease management, and other skilled nursing interventions within the RN's scope of practice.
• Complete OASIS assessments and maintain accurate, timely clinical documentation in the agency's electronic medical record system.
• Monitor patients' conditions and report significant changes to the physician and clinical supervisor.
• Educate patients and caregivers about medications, treatment plans, disease management, and home safety.
• Coordinate care with physicians, therapists, patients, caregivers, and agency staff.
• Follow applicable Florida regulations, agency policies, physician orders, and home health documentation requirements.
• Communicate effectively in English and Spanish with patients and families.
• Travel to assigned patients' homes within the agreed-upon service area.

Qualifications

• Active, unrestricted Florida Registered Nurse (RN) license.
• Bilingual in English and Spanish required.
• Home health experience preferred.
• OASIS assessment and documentation experience preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and willingness to travel to assigned patients.
• Current CPR certification and completion of applicable agency onboarding requirements.
• Ability to complete electronic medical record documentation accurately and on time.
