**Job Summary:**

We are seeking a skilled and motivated **Physical Therapist (PT)** to join our dynamic rehab team. In this role, you will assess, plan, and implement individualized therapy programs for both short\-term rehab patients and long\-term care residents. The ideal candidate is a caring professional who enjoys working in a team environment and is committed to helping individuals regain or maintain their independence in daily living

**Key Responsibilities:**

• Evaluate patients’ functional abilities and develop individualized treatment plans
• Provide hands\-on therapy to improve residents’ and patients’ abilities
• Document progress and adjust treatment plans as needed
• Educate patients, residents, and caregivers on exercises and strategies to improve function
• Collaborate with interdisciplinary team members, including physical therapists, speech therapists, nursing staff, and social workers
• Ensure compliance with regulatory requirements and documentation standards
• Maintain accurate records using electronic medical record (EMR) systems

**Qualifications:**

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program
• Current state licensure as an Physical Therapist (PT)
• Excellent communication, organization, and time management skills
• Compassionate, patient\-centered mindset with a passion for helping others
