Entera Healthcare is seeking a compassionate and motivated Physical Therapist (PT) to join our growing Home Health team serving Ottawa and surrounding communities.

Responsibilities
- Evaluate and treat home health patients
- Develop individualized treatment plans
- Document patient progress
- Collaborate with physicians, nurses, and interdisciplinary team members
- Promote patient safety, mobility, and independence
Qualifications
- Active Illinois Physical Therapist License
- Home Health experience preferred but not required
- Reliable transportation
- Current driver's license and auto insurance
- Excellent communication skills
