Summary

As a Registered Nurse - Home Health at Enliven Home Health, you will play a vital role in delivering personalized nursing care to patients in their homes. This position is essential for ensuring optimal health outcomes and enhancing the quality of life for our clients.

Hartford County including cities

West Hartford, Rocky Hill, South Windsor and surrounding towns

Responsibilities

• Provide direct patient care in accordance with established care plans.
• Conduct comprehensive assessments of patients' health status and needs.
• Develop and implement individualized nursing care plans.
• Coordinate with healthcare providers and family members to ensure continuity of care.
• Educate patients and their families on health management and disease prevention.
• Monitor patient progress and adjust care plans as necessary.
• Document all patient interactions and maintain accurate medical records.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Experience in home health or community-based nursing preferred.
• Strong knowledge of case management and discharge planning processes.
• Ability to work independently and manage time effectively.
• Excellent communication skills for interacting with patients, families, and healthcare teams.
