**Overview**

Outpatient, One on one multidisciplinary practice

**Duties**

  • Develop and implement comprehensive treatment plans tailored to individual patient needs across various specialties such as orthopedics, neurology, pediatrics, and geriatrics.
  • Lead and supervise a team of physical therapists and support staff to ensure high\-quality patient care aligned with clinical standards.
  • Conduct detailed assessments of patients’ physical conditions, including mobility, strength, and functional abilities, utilizing your knowledge of anatomy and physiology.
  • Manage discharge planning processes by coordinating with healthcare professionals and families to ensure smooth transitions from hospital to home or community settings.
  • Maintain accurate clinical documentation in electronic health records (EHR) systems according to healthcare regulations and standards.
  • Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to develop integrated care plans.
  • Oversee patient rehabilitation programs focusing on outpatient care, inpatient recovery, home health services, and outpatient rehabilitation centers.
  • Ensure compliance with all clinical documentation standards and healthcare policies while fostering a culture of continuous improvement in patient care practices.

**Skills**

  • Extensive experience in acute care, inpatient hospital settings, outpatient clinics, or home health environments with a focus on patient rehabilitation.
  • Strong knowledge of medical terminology, physiology, anatomy, and physiotherapy techniques such as trigger point therapy.
  • Proven ability to develop effective treatment plans based on thorough assessments of patients’ conditions.
  • Excellent client communication skills to educate patients and families about treatment processes and progress.
  • Skilled in discharge planning and working with geriatric patients to promote independence and quality of life.
  • Proficiency in electronic health records (EHR) management systems like EMR platforms for accurate documentation.
  • Ability to collaborate effectively with healthcare professionals across disciplines to deliver holistic patient care.
  • Experience working with pediatric populations and in outpatient rehabilitation centers is highly desirable.

Pay: $50\. 00 \- $55\. 00 per hour
